When you have completed the following forms, a request for access will be sent to the appointed lead for your hospital to review. Once approved, you will receive an email confirming access to the platform, so please ensure the email address you choose is one that you use frequently.
Please note, the process of verifying your registration with your hospital can take some time, and up to several weeks depending on the availability of the appointed lead for your hospital.
